Request Support or Nominate Someone in Need

The form below can be used to request funds from the Northern Colorado FOP Lodge #3 Benevolent Fund. You must meet the criteria of the Benevolent Fund to be considered for support. A general definition is that you must be a Law Enforcement employee or a family member with a financial need. Anyone may make the request for funds, but the above criteria must be met. Please contact the lodge if you have any questions not answered here.
